Abstract
The article provides a detailed analysis of the clinical indications for root end resection (apicoectomy) in cases where conservative root canal treatment has failed. It examines anatomical variations of teeth, such as canal curvature, and the presence of large cysts and granulomas that prevent standard retreatment. The author explains the economic and practical feasibility of the surgery when orthopedic structures (crowns and posts) are present, allowing the tooth to be preserved and avoiding extraction.
